Glendale Adventist Medical Center will be participating in the ASHP PhORCAS service (available Fall 2012) and all documents below will need to be submitted via this service. For more information about PhORCAS, please visit www.ashp.org/phorcas.
Application materials include:
- Letter of intent
- Official transcripts
- Curriculum vitae
- Three letters of recommendation (at least one letter from a clinical rotation experience).
Eligible candidates will have a Doctor of Pharmacy degree from an accredited school or college of pharmacy. An on-site interview is required. All application materials are due by January 11, 2013.
