Regardless of your statement type, please pay close attention to the "Important Message" box on the top right of your bill. Our Patient Business Office may use it to provide information or ask you to:

A Statement When We Contact Your Insurance Company
If you have health insurance, we will bill your health plan and send you a statement. You should only pay your deductible and co-insurance at this time.
A Statement Requesting Payment
Once your insurance company has processed your claim, we will send you a statement. You must them pay any unpaid balance. Click here to see a sample of a GAMC statement requesting payment.
If you do not have insurance, the first statement you receive is your bill.
Full payment is due when you receive a bill requesting payment. If you need to make other financial arrangements, please contact our Customer Service Department by calling (818) 409-8200 or by completing a Patient Financial Services help form.
