Medical Benefits

Glendale Adventist Medical Center is a self-insured facility that provides employees with a medical plan that covers a wide range of medical services.

Exclusive Care (EPO)

  • Employees may select a primary care physician from a roster of approved providers.
  • Referrals to specialists are authorized by your primary care physician and approved by Claims Central Utilization Review.
  • There is a $15 co-payment for office visits.
  • For inpatient and outpatient hospitalization, the plan pays 90% and you pay 10%, to a maximum of $1,000 out-of-pocket.
  • Medical plan is $28/month for full-time employees and $112/month for part-time employees.

Dental Plans

Employees have three dental plans to choose from:

Adventist Health PPO Dental Plan
Allows you to select any dental provider of your choice. Coverage includes:

  • Preventative Services covered at 100%, twice per calendar year
  • Basic Services covered at 80%
  • Major Services covered at 50%

Cigna Dental HMO
Requires you to select a dental provider from their contracted panel. A schedule of benefits is available upon request.

Safeguard Dental HMO
Requires you to select a dental provider from their contracted panel. A schedule of benefits is available upon request.

Vision Care

Vision care is included with your medical plan. The vision plan pays 80% up to a $300 maximum for a two-year period.

Prescriptions

Prescriptions are processed through Express Scripts. The co-payments are as follows:

  • Generic: $10
  • Brand: $20
  • Non-Formulary: $40

Retirement Plan

  • 401(a) Qualified Plan-Basic Contribution
  • 401(a) Qualified Plan-Matching Contribution Employer
  • 403(b) Tax Sheltered Annuity Plan

Employee Assistance Program

Glendale Adventist Medical Center has contracted with Adventist Health Behavioral Medicine to provide you and your family with guidance, focus and support for a wide range of personal and work-related problems that may otherwise interfere with your daily lives. Six free sessions are available to each family member per year.

Life Insurance

A $10,000 policy is provided by Glendale Adventist Medical Center. A voluntary plan is also available.

Accident Insurance

A $10,000 policy is provided by Glendale Adventist Medical Center. A voluntary plan is also available.

Childcare

On-site childcare is available for employees at a discounted rate of $3/hour and can be paid through payroll deduction. Mildly ill, back up, weekend and evening childcare is also available.

Paid Leave Policy

Paid leave is available for employees who work at least 48 hours per two-week pay period. An employee who works 80 hours per pay period receives:

Vacation time

  • Eight paid holidays per year
  • 12.15 disability days per year

Official Holidays

Employees who work on the official holidays will be paid a holiday premium.

Tuition Assistance

Tuition assistance is available for full-time employees who have been employed at GAMC for a minimum of one year. An approval process is in place for this benefit.

Alternative Transportation, Commuter Program

Glendale Adventist Medical Center provides transportation subsidies to those employees who take the bus or travel on the Metrolink. Please contact Human Resources at 818-409-8040 for details.
